Community Child Health Team/Care Coordination Team

The CHSC Community Child Health Team (CCHT) works to promote the spread of the medical home model among community-based primary care providers, especially for the purpose of improving quality of care for children and youth with special health care needs. CHSC shares its special expertise in family-centered, care coordination services with primary care and specialty care providers. We help deliver care coordination and family support services within the primary care practice. We facilitate connections amongst medical, educational, and community agencies.
