Family-to-Family Iowa

CHSC is leading a project to meet the needs of families of children and youth with special health care needs for more accessible information and resources; more available family-to-family support; and more meaningful family-professional partnerships. Family-to-Family Iowa is guided by strong multiagency collaboration, family-driven planning, and effective use of program evaluation. The ultimate goal is to have a cohesive, coordinated, culturally competent statewide network of family mentors/educators to serve families of children and youth with special health care needs. This project expands on CHSC’s already talented, committed team of family navigators by partnering with additional family advocacy groups throughout Iowa. Family to Family Iowa is supported from two funding sources: 1) funds from the Health Resources Services Administration for Family to Family Health Information Centers and 2) funds from the Administration on Children and Families/Health and Human Services, supporting the Family 360 Project.
